Immunization Requirements

 IMPORTANT NOTICE TO PARENTS and GUARDIANS OF SIXTH GRADERS
 

The Massachusetts Department of Public Health has the following requirements for all students entering Grade 7 in the fall:

  1. PHYSICAL EXAM: A copy of a current Physical Examination (within 12 months of the first day of the school year) is required for all students before entering Grade 7. A current Physical Examination (dated within 13 months) will also be required for any participation in sports at the Junior High.

 

  1. IMMUNIZATION REQUIREMENTS (link to MA School Immunization Requirements):
  • Tdap: 1 dose; and history of DTaP primary series or age-appropriate catch-up vaccination; Tdap given at ≥7 years may be counted, but a dose at age 11–12 is recommended if Tdap was given earlier as part of a catch-up schedule; Td or Tdap should be given if it has been ≥10 years since last Tdap
  • Polio: 4 doses; fourth dose must be given on or after the 4th birthday and ≥6 months after the previous dose or a fifth dose is required; 3 doses are acceptable if the third dose is given on or after the 4th birthday and ≥6 months after the previous dose
  • Hepatitis B: 3 doses; laboratory evidence of immunity acceptable; 2 doses of Heplisav-B given on or after 18 years of age are acceptable
  • MMR: 2 doses; first dose must be given on or after the 1st birthday, and second dose must be given ≥28 days after first dose; laboratory evidence of immunity acceptable
  • Varicella: 2 doses; first dose must be given on or after the 1st birthday and second dose must be given ≥28 days after first dose; a reliable history of chickenpox* or laboratory evidence of immunity acceptable
  • Meningococcal: 1 dose; this dose must be given on or after the 10th birthday. Meningococcal conjugate vaccine, MenACWY (formerly MCV4) and MenABCWY, fulfill this requirement; monovalent meningococcal B (MenB) vaccine is not required and does not meet this requirement.

Many of the above vaccines may have been administered while your child was in elementary school. Please check with your Healthcare Provider to be certain that your child’s immunizations are up-to-date and documented in their record. Send a copy of the exam and updated immunization record to your elementary school health office, and the information will be included in your child’s health record when it is transferred to the Junior High School in June. If you do not have the necessary paperwork completed before the end of school, send it to the school nurse at the Junior High School (16 Charter Road) before the start of school in August.
